검사의 목적

류마티스 관절염 진단과 다른 유형의 관절염과의 구별하는 것을 돕기 위해; 때로는 류마티스 관절염 환자의 예후를 평가할 수 있도록 돕기 위해 검사를 시행합니다. 

검사가 필요한 경우

의사가 관절 염증을 가진 환자에서 또는 진단이 어렵거나 감별이 어려운 염증성 관절염을 가진 환자에서 류마티스 관절염을 의심하는 경우, 증상이 있지만 류마티스 관절염의 진단 기준을 아직 만족시키지 못하는 경우 검사를 시행합니다.

어떤 검사입니까?

순환 시트룰리네이트 펩티드 항체(CCP) 검사는 혈액에서 시트룰린 항체의 존재를 검출하는 비교적 새로운 검사 방법입니다. 이러한 자가항체는 시트룰린을 항원으로 인지하여 면역 시스템에 의해 생성되는 단백입니다. 비정상적인 아미노산인 시트룰린은 아미노산인 아르기닌이 변경되었을 때 생성됩니다. 아르기닌에서 시트룰린으로의 전환이 류마티스 관절염(RA) 환자의 관절에서 관찰되는 자가 면역성 염증 과정에 역할을 하는 것으로 추측되고 있습니다.

류마티스 관절염(RA)은 손, 발, 그리고 전신에 걸쳐 다른 관절에 염증, 통증, 강직, 그리고 파괴적인 변화를 일으키는 만성 전신성 자가 면역 질환입니다. RA의 합병증을 최소화하기 위한 여러 가지 치료가 있지만, 정확한 진단과 상당한 관절 손상의 진행되기 전에 치료를 시작하느냐가 중요합니다. 류마티스 인자(RF)는 RA를 감지하고 다른 유형의 관절염 및 기타 염증성 진행 과정으로부터의 감별을 위해 사용되는 기본적인 혈액 검사로 시행되어 왔습니다. 그러나, RF의 민감도 및 특이도가 이상적으로 우수하지 않아 RA의 임상 징후를 보이는 환자에게서 음성을 보이거나 그렇지 않은 환자에게서 양성을 보이기도 합니다. 연구들에 의하면 CCP는 민감도 및 특이도가 RF 보다 동등하거나 우수하고 RF보다 훨씬 더 조기에 RA 환자에게서 양성을 보이는 것으로 나타났습니다.

미국 류마티스 학회에 따르면, CCP 항체는 증상의 시작 후 3~6개월 만에 초기 류마티스 환자의 약 50~60%에서 검출됩니다. RA의 조기 발견과 진단으로 의사들은 연관된 합병증과 조직의 손상을 최소화할 수 있도록 적극적인 치료를 시작할 수 있게 됩니다.

검사의 해석 및 흔한 질문들
  • 검사 결과는 어떻게 활용됩니까?

    CCP 검사는 류마티스 인자(RF)와 함께 검사 하거나 추가로 시행하여 류마티스 관절염 진단에 도움을 줍니다. CCP 검사는 감별이 어려운 관절염 환자-이들의 증상이 류마티스 관절염의 ACR 진단 기준에 부합하지 않는-에서 RA 발생 가능성이 높은지를 평가하기 위해 보조적으로 시행될 수 있습니다. 미국 류마티스 학회에 따르면, CCP 양성 환자의 약 95 %에서 향후 RA가 발생할 것이라고 합니다.

     

  • 검사는 언제 하게 됩니까?

    CCP검사는 이전에 진단되지 않은 염증성 관절염을 앓고 있거나 감별이 되지 않는 관절염으로 진단이 된 환자에서 주로 RF 검사와 함께 기본적으로 시행됩니다. 대칭적인 관절 통증과 염증 등의 임상 증상이 있어 의사가 RA를 의심할 때 RF 검사가 음성을 보이는 경우 후속 검사로 시행될 수 있습니다.

  • 검사 결과의 의미는 무엇입니까?

    원칙적으로, 임상 징후와 증상의 맥락을 고려하지 않고 검사 결과를 판단할 수 없습니다. 그럼에도 불구하고 CCP 및 RF에 모두 양성을 보이는 환자들은 RA일 가능성이 높고, 더 심한 형태로 발전할 수 있습니다. 환자들이 CCP 양성, RF 음성 결과를 보이면서 임상 징후가 RA를 시사한다면 향후 RA가 발생하거나 조기 RA일 가능성이 있음을 시사합니다.

    환자들이 CCP 음성, RF 양성 결과를 보인다면, 임상 증상 및 증후는 RA인지 다른 염증성질환 인지를 결정하는데 매우 중요합니다. 환자들이 CCP 및 RF 모두 음성 결과를 보인다면, RA가 있을 가능성이 적습니다. 하지만 RA는 임상적으로 진단되는 질환임을 염두 해야 하며, 따라서, 자가 항체 검사 양성 결과 없이도 진단이 가능합니다. 

  • 그 외 중요한 것이 있습니까?

    CCP 검사는 유망하고 더 보편적으로 사용되고 있지만, 궁극적인 임상적 유용성과 이용 방법이 완전히 결정되지는 않았습니다.

  • 모든 사람에 대해 CCP 검사를 해야 하나요?

    아닙니다. CCP 검사는 선별검사로 추천되지 않습니다. RF 검사처럼 임상 증상이 RA를 시사하는 경우나 감별되지 않는 관절염으로 이미 진단된 환자를 대상으로 시행합니다.
